University Operations Services requires the completion of this form for all new Dorm Crew employees. Please read the following instructions before filling out the form. (There is a link to the form at the bottom of this page.)
- The TR is a Word file that should be saved and filled out on your computer, and then emailed to the Dorm Crew Office. Please see below for instructions about the subject line and email address you should use for this email.
- Use the Tab button or your mouse to move from field to field, and type in your answers. When you have completed the form,
- Go to View » Zoom to zoom in and make the form easier to read.
- After entering the requested information, please save the file on your computer and email it to us as an attachment. If you are signing up for term-time work, please email the attachment to dorm_crew@harvard.edu with “Term-Time Temp Reg” as the subject. If you are signing up for storage work, please email the attachment to dorm_crew@harvard.edu with “Storage Temp Reg” as the subject. If you are signing up for Spring Clean-Up, please email the attachment to dormcrew1@harvard.edu with “SCU Temp Reg” as the subject. If you are signing up for Fall Clean-Up, please email the attachment to dorm_crew2@harvard.edu with “FCU Temp Reg” as the subject.
- While filling out the form, keep the following points in mind:
- Working for libraries, academic departments, the Bureau of Study Counsel, the athletic department, etc. count as Harvard employment, but working for the Harvard Student Agencies (HSA) does not.
- The fields Department, Supervisor’s Name and Dates of Service refer to past or current Harvard employment.
- You can sign this form by simply typing your name and the date in the appropriate fields.
