I-9 (Employment Eligibility Verification) Form
Federal regulations require the completion of this form for all new employees. Please read the following instructions before filling out the form. (There is a link to the form at the bottom of this page.)
Notes for everyone:
- You need Adobe Reader 5.0, or better, to fill out this form. The newest free version of Adobe Reader can be downloaded here, or you can simply use a computer in your House computer lab or in the Science Center.
- The I-9 is a PDF form that can be filled out on your computer. Use your mouse to move from field to field, and type in your answers. (Note that if you are using just Acrobat Reader and not the full Adobe Acrobat software, you will only be able to save the original form, and not your answers.)
- You only need to fill out the top third of the form (Section 1 above the Employee’s Signature line).
- After entering the requested information, please print out the form on white paper. Then sign and date it with black or blue ink, and bring it to the Dorm Crew Office. If you are a U.S. citizen, please make sure to bring your passport OR both your social security card and Harvard ID with you. If you are an international student or a permanent resident, please see the instructions below.
Notes for foreign nationals:
- Check the “An alien authorized to work until” box. You are authorized to work until the date in Block 5 of your I-20 form. Your Alien/Admission number is 11 digits long and is listed on the top of your I-94 form (usually stapled into your passport).
- When you bring your completed and signed I-9 to the Dorm Crew Office, remember to bring your printed and completed Temporary Registration, M-4, W-4, and FNIF forms, along with your I-94 card, I-20 form, and unexpired passport with your visa.
Notes for Permanent Residents:
- Check the A Lawful Permanent Resident box. Make sure to enter your Alien number from your Permanent Resident Card (or Resident Alien Card, Alien Registration Receipt Card, or Form I-551).
- When you bring your completed and signed I-9 to the Dorm Crew Office, remember to bring your unexpired Permanent Resident Card.
